Important information
- Patients must be clinically stable and in a safe home environment to be eligible for admission to Vitalis clinical services.
- HITH and Complex Care patients can be admitted under the care of their treating doctor or under Vitalis’ own GP/Emergency Doctors.
- Chemotherapy at Home patients are admitted under the care of their treating doctor.
- Treating doctors are consulted and provided with regular updates.

How do I refer to Vitalis for Hospital at Home services?
There are a number of ways of referring to Vitalis, depending on the service you are looking to book.
Hospital in the Home (HITH), Rehabilitation in the Home (RITH) and Complex Care in the Home services can be booked using any of the methods (online, email and fax) listed above.
For all other services, please call 1300 848 254 or contact us.
What areas does Vitalis cover?
Vitalis provides Hospital at Home and Care at Home services across the Greater Sydney region. We may also be able to support patients in other locations. Please contact us to discuss specific cases.
How long does the referral process take?
Once a referral form has been received, a member of the relevant team will be in touch with you on the same working day (generally within an hour). For Hospital in the Home (HITH) patients, the Vitalis team is generally able to start providing care on the next working day (assuming the patient is eligible to receive care and subject to circumstances). For Rehabilitation in the Home (RITH) patients, the Vitalis team is able to start providing care within 72 hours.
Can I refer myself to Vitalis as a private patient?
