Important information
- If referring for IV antibiotics, please attach Med Chart, PICC/Port insertion report and recent blood results where indicated on the form.
- If referring for negative wound therapy (VAC), please attach Surgical Report or Wound Chart with dimensions/regime where indicated on the form.
- Patients must be clinically stable and in a safe home environment to be eligible for admission to Vitalis clinical services.
- HITH and Infusion Therapy patients can be admitted under the care of their treating doctor or under Vitalis’ own GP/emergency doctors.
- Chemotherapy at Home patients are admitted under the care of their treating doctor.
- Treating doctors are consulted and provided with regular updates throughout the treatment.

How long does the HITH referral process take?
Once a referral form has been received, a member of the relevant team will be in touch with you on the same working day (generally within an hour). For Hospital in the Home patients, the Vitalis team is generally able to start providing care on the next working day (assuming the patient is eligible to receive care and subject to circumstances).
